 Speak with your professor  If your professor is able to accommodate you for your exam, there is no need to submit a form for that class.

 If you have a conflict caused by extended time on exam, discuss alternate times ▪ Exam dates and times should be exactly the same as your class unless otherwise discussed with your professor.

▪ Any date/time that differs from the professor’s schedule requires professor approval.

 “Save and Sign as Completed”  Sends professor email requesting they enter necessary info.

 “Save and Return Later to Finish,”  Saves the info but will NOT submit  NOT on the schedule with ODS  “Cancel”  Will clear any info entered and not submitted

 ODS will deny the request if:    Does not provide 9 days’ notice Does not provide a exam time Requested on weekend or at an unapproved time  At such time, you will need to take your exam with class.

 If your status reads:  Waiting on Student, Waiting on Instructor ▪ You are not quite done. ▪ Please either submit your form or politely ask your instructor to enter their information.

 Waiting on Office of Disability Services ▪ No additional action is needed unless ODS denies form or requests additional information.

 Form is Complete ▪ Ensure all dates are approved

Upcoming Exams Needs To Be Submitted—Press The “Save And Sign As Complete” Button Politely Remind Your Instructors To Enter Exam Information Review to see if requests are approved.

 No Shows   Do not arrive for exam + No communication = No Show Receive email reminding you of scheduled exam with our office & of the ODS “no show” policy.  2 nd “No Show” for the same course  Dropped from scheduled for that course for remainder of scheduled exams for current term

 Academic Integrity violations will be reported to instructors.  All cell phones, notes and other non-exam related materials should be left in your backpack.

 To keep track of time, be sure to bring a watch.  Not all classrooms on campus have functioning clocks.
