Based on the information in the story below translate language
into medical terminology when possible and use shorthand
when possible. Divide info into history and observation
• Athlete was playing soccer and she was on the wrong end of a slide
tackle. She felt the opponent’s leg crash into the outside of her
right leg. She felt immediate pain in the lower portion on the
outside of her ankle. There was an inside turn to the ankle, a pop
was felt, the pain was at the upper end of her tolerance level.
There was a concentrated amount of swelling on the outside of the
ankle. She couldn’t bring her ankle up towards her head very well,
she could point it well, and turning towards the inside increased her
pain the most. She felt some weakness in the muscles on the
outside of the leg and could feel in when she put pressure on the
ankle that side to side movements were wobbly. She didn’t feel any
tingling or numbness. When she walks she needs assistance as it
seems to intensify her pain. This is her first injury.
Write a SOAP Note for the following injury scenarios.
Write concisely using medical terminology and
shorthand when possible. Use your imagination and be
creative.
