Definition/Purpose:
Granted for superior efforts/outcomes in
accomplishing a program’s mission; usually awarded
to NPAC subcommittee members after 2-3 years of
service for sustained above average effort; considered
a higher level of award than the Letter of Achievement
Documents needed for submission:
Co-chairs will submit a list
Submit to: Special Recognition Team Leaders
Time frame: Call for submission in June or
July, deadline is August 1

Definition/Purpose:
Granted in recognition of a specific noteworthy
achievement; usually awarded to NPAC subcommittee
members after 1 year of service and completion of
work assigned
Documents needed for submission:
Co-chairs will submit a list
Submit to: Special Recognition Team Leaders
Time frame: Call for submission in June or
July, deadline is August 1

Definition/Purpose:
Any officer officially detailed to a special program
initiative of a Federal, State, or other agency that is not a
routine function of that officer’s position for a minimum
of 30 consecutive days (includes NPAC voting members
completing the 3-year term)
Documents needed for submission:
Copy of the Surgeon General’s appointment letter
and NPAC Awards Tracking Sheet
Submit to: Special Recognition Team Leaders
Time frame: Call for submission in June or
July, deadline is August 1

Definition/Purpose: Awarded
at the sole discretion of CNO
for exemplary contributions to
initiatives in support of the
mission of the NPAC

Definition/Purpose:
Awarded to an officer in recognition of a specific
and noteworthy achievement for a short time period

Narrative, completed PHS Form 6342-2, and NPAC

Call for submission in June or July, deadline is
August 1

Definition/Purpose:
Awarded to an officer for superior efforts or
outcomes in accomplishing a program’s mission
(typical level of award for NPAC Chair)

Narrative, completed PHS Form 6342-2, and NPAC

Time frame:
Call for submission in June or July, deadline is

Definition/Purpose:
Awarded to an officer who has exhibited a level of
proficiency and dedication distinctly greater than
expected
Documents needed for submission:
Narrative, completed PHS Form 6342-2, and NPAC

Call for submission in June or July, deadline is
August 1

Definition/Purpose:
Granted to acknowledge significant contributions
and achievements well above that normally expected
in accomplishing the goals and objectives of the
OPDIV/STAFFDIV or non-HHS agency
Documents needed for submission:
Narrative, completed PHS Form 6342-1, and NPAC
Awards Tracking Sheet
Submit to: Agency PHS Liaison
Time frame:
Must submit within 13 months of the noteworthy
act or period to be recognized; call for submission in
June or July, deadline is August 1

Definition/Purpose:
Granted to a group of officers who exhibit
outstanding contributions and achievements well
above that normally expected in accomplishing the
goals and objectives of the OPDIV/STAFFDIV or nonHHS agency
Documents needed for submission:
Narrative, completed PHS Form 6342-1, and NPAC
Awards Tracking Sheet
Submit to: Agency PHS Liaison
Time frame:
Must submit within 13 months of the noteworthy
act or period to be recognized; call for submission in
June or July, deadline is August 1

Narrative should we well written, highlight the accomplishment(s) and impact, be clear and concise (< two single
spaced typed pages with 1” margins and 12-point font,
Times New Roman or comparable font with no more than
12 characters per inch)
Plan to submit awards documentation well in advance to
accommodate the review process
Work with agency PHS Liaison through entire awards
submission process
Take the initiative to recognize, document, and submit
your accomplishments and those of others
