Beta Test Letter of Invitation
• 29 health departments (8 state;19 local
and 2 tribal)
• Any organizational size and
configuration
• Joint applications will be accepted
• Will not test state-based accreditation or
multi-state regional – to be tested later

• What Beta Test Sites Must Do:
– Select an accreditation coordinator
– Attend training session
– Complete an online application form
– Complete a self assessment
– Gather and submit documentation
– Host an on-site visit
– Gather time and cost information
