Membership Process
Member must be a Provista pharmacy participant to access contract, if
not, DEA number needs to be provided to membership to activate.
Once pharmacy participant, the following steps need to be taken to
access the program:
• Submit current vaccine purchase history to your Provista Account Executive
• Complete new member request form and send to Dena Kitchens
([email protected]) or Kelley Charlton, NAM Sanofi Pasteur
([email protected])
• Once approved by Provista and Sanofi Pasteur, member should complete the LOC
available in form manager
Member compliance is monitored on a monthly basis
Member Requirements
The Group (in aggregate) as well as EACH MEMBER must
maintain Sanofi-Pasteur compliance of greater or equal to
the market share commitment on the following product
lines:
• 90% market share of Pediatric category vaccines:
PENTACEL, IPOL(Polio), ACTHIB (Hib), DAPTACEL (Pertussis)
• 90% of prior year’s purchases of TdaP (ADACEL or
Boostrix).
• ADACEL must be the TdaP vaccine used under the
Vacce$$ Program
• 80% of prior year’s purchases of Meninge (Menactra or
Menveo).
• Menactra must be the Meninge vaccine used under the
Vacce$$ Program
Terms and Conditions
Vacce$$ is a
committed
program
• If member is non-compliant they will be removed from
the program
Facility
conversion
time limited
• Ramp up period of no more than four months
• Please begin the conversion process quickly to
ensure purchases are reflected prior to the 4 month
requirement
WHY?
• The Vacce$$ program aggregates all participating

• Member non-compliance may risk other customers
of losing their Vacce$$ contract benefits
