Select
Application Tutorial

Overview
While searching for a provider that is right for you, the Provider
Search and PCP Select application allows you to compare
providers, view driving directions and maps, view details, and
create a provider notebook containing addresses and personalized
notes.
• This tutorial will walk you through the capabilities of the Provider Search and PCP
Select application.
• The following diagram provides an overview of the tutorial’s flow from page to page.
The links on the diagram can be used to navigate directly to a specific page found in
this tutorial.
• A smaller version of this diagram can be seen throughout the tutorial in the upper lefthand corner with one region shaded, in order to note which page you are on in the
process. To navigate forwards and backwards in this tutorial, click on the appropriate
chevron in the upper left-hand corner of the following pages to jump to the desired

Provider Search
The Provider Search page allows you to select search parameters.
To view what is new with
the current version of the
application, click the
What’s New? link.
(Available on all pages)
Enter information.
To view the workflow and
topics that may be of help to
you throughout the process
of finding a provider, click
the Help link. (Available on
all pages)
Note: This menu only
displays for the HAP HMO
and Senior Plus plans.
To enter additional search
information to narrow your search,
click the Additional Search
Criteria button.
To begin your search, click
the Find Providers Now!
button.

Additional Search Criteria
The Additional Search Criteria page allows you to narrow your search by distance, specialty type or other
optional search parameters.
To find a ZIP code, click the
Zip Code Lookup link.
If searching by distance, you
must enter your ZIP Code
and Distance in miles that
you prefer to travel.
Enter information to
narrow your search.
This dropdown only appears
when you choose PCP as the
Type of Provider and a Plan
type which has hospital
affiliations. When
searching by quality, an
additional “Minimum Level
of Quality” drop down
appears.
To begin your search, click the Find
Providers Now! button.

Your Search Results
The Your Search Results page contains a list of providers obtained from your search criteria.
The ZIP code shown here and the
distance column on the table are only
displayed when you search by
location.
To narrow your search and go
to the Additional Search
Criteria page, click the Narrow
Down Search link.
To view the next providers
listed, click here. After
clicking, a <<Previous
link displays to be able to
return to the first providers
listed.
To view detailed information about
the provider or facility, click the
provider’s or facility’s name.
To compare up to three providers,
select up to three boxes and click the
Compare button.
To return to the Provider
Search page to begin a new
search, click the New Search
button.
To send search results in an Adobe
file via email, click the Create
Personalized Directory link.

Provider Comparison
The Provider Comparison page allows you to compare up to three
providers or facilities at one time.
If the provider is a PCP,
clicking the Download
performance data for HAP’s
physician hospital affiliations
link opens a PDF that displays
hospital affiliation
performance data. If the
provider is a hospital, external
links are listed to search for
hospital performance data.
To return to the Your Search
Results page, click the Back
To Results button.
To search if sanctions are on
record with the state of
Michigan for a provider, click
the Michigan Department of
Community Health link.
To view detailed information
about the provider, click the
provider’s name.
View the provider information
that is compared.

Provider Details
The Provider Details page allows you to read information about a provider or facility
and gives you the opportunity to add the provider or facility to your Provider Notebook.
To view maps and
driving directions,
click the Get Map
link.
Click here to add to your Provider Notebook.
Click Add Notes to record your personal
remarks about the provider. Click Modify
Notes to update your remarks.
To view all the hospitals to
which this provider has admitting
privileges, click the Show
Hospitals and Quality Data
link.
Clicking the Show Specialists link displays a list of
specialists that the PCP may refer to. Clicking the Show
PCP’s link displays a list of PCPs that the Specialist may
refer to.
Hospitals have a quality section
from which you can access the
Leapfrog Group’s or the Greater
Detroit Area Health Council’s web
sites to search for hospital quality
and performance data.
This table displays quality
information for a PCP’s hospital
affiliation(s). If the PCP has
multiple affiliations, you may
select one from the drop down to
display its results.
Clicking the Select PCP link allows you
to designate the provider as your PCP. If
you are a Guest, you will be prompted to
log in or register first.

Member Login or Registration
The Member Login or Registration page allows you if you are not logged in, to log
in or register if you have not yet registered for online services. After successfully
logging in from this page you are able to select or change your PCP to the Provider
you were viewing.
Enter your ID Number
and Password if you are
a registered online HAP
Member.
To navigate to the Select
PCP Page to select or
change your PCP, click the
Log In button.
To register for online services if
you are a Member, click the
Register for Online Services
link.
To receive advice if you forgot
your ID Number and/or
Password, click the
Login/Password link.

Select PCP
The Select PCP page allows you to select or change your PCP. You must be a
registered online HAP Member to perform this action.
To select the
appropriate Member to
have this Provider as
their PCP, click on the
checkbox.
After you select the appropriate
Member and select an option
from the dropdown, click the
Confirm button to finalize your
PCP choice.
Select a reason for
changing your PCP
from the dropdown box.

My Provider Notebook
The My Provider Notebook page allows members and providers to save information
about your favorite providers or facilities. You may also add, view, or modify personal
notes. This page is only accessible from the left navigation bar.
To print, click the Printer
Friendly button.
To remove a provider, click
on the box and click Update
Notebook.
To add, view, or modify
notes, click either the Add
Notes link or the
View/Modify Notes link.

My Provider Notes
The My Provider Notes page allows members and providers to add,
view, or modify personal notes about a specific provider.
Add or modify
your notes here.
This provider has a long wait
time.
To save your notes and
return to the My Provider
Notebook page, click the
Update button.
