(CHCS) Establish a Method to Measure Progress • Should directly measure the written goal • Can be rapid-cycle with small samples • Should be measured over time across a wide range of variables/conditions • Should differ from the HEDIS measure you are trying to improve Establish a Method to Measure Progress Example of Measurement of Improvement: Numerator: • Number of successful outreach attempts to members who are due for a postpartum visit Denominator: • Total number of members who are due for a postpartum visit Implement Change • Identify who, what, where, and when • Sufficient training and communication are essential • Pilot test the change using a smaller group to readjust plans as needed. Consider multiple pilot tests to evaluate adjustments made.
