Integrated Assessment and Single Plan Introduction

All children and young people meeting the CYPDS Eligibility Criteria
will be offered the new IA and SP from June 2012 (approx 60-80
families from June 2012 – March 2013)

Single route of entry through ‘application’ to the CYPDS Decision

The ‘application’ can be made by a professional, with parental consent
or can be made by parents directly

Assessment Co-ordinator: convenes initial Assessment Team Meeting (must
include representation from Education, Health and Social Care) and co-ordinates
Integrated Assessment timetable
Assessment Team: collates assessment information. As a minimum this
assessment information should include information from:
•The child/young person
•The parent/carer
•The Community Paediatrician and any other relevant medical practitioner
•Educational Psychology
•The school or pre-school (if the child is attending a school or pre-school)
•Social Care (even if the information available is that the child/young person is not
known to social care)
•Any Specialist Teachers involved in supporting the child/young person
•Any Therapists involved in supporting the child or young person
•Information from the Health Visitor / School Nurse about the outcome of routine
health surveillance checks
Where possible, the Assessment Team should meet during the information
collation period to discuss progress and propose interim interventions if necessary.
Parents/Carers and their child/young person are updated regularly on progress,
findings and giving their views.
Wider Professionals’ meeting: convened by the Assessment Co-ordinator to
collectively analyse assessment findings; propose Single Plan and allocate Lead

Personal Budgets Introduction

Personal Budgets Pilots in Southampton will give families, children and young
people more control about what education, health and social care services are
used to support them.

Pilot projects that will run as part of the Pathfinder:



Continuing Health Care Personal Budget Project



Home to School Transport Direct Payments
Single Plan Notional Budgets with offer of some Direct Payments (including Short
Breaks)
Continence & Equipment Personal Budgets
Individual Education Direct Payments
The aim of the pilots are to ensure:



that Pathfinder families know and understand their funding allocation
its clear why that amount has been allocated
families are directly involved in deciding how funding is used

Vulnerable Children
 The scope of the integrated assessment and single plan:
What will the eligibility criteria be for the defined group of
vulnerable children to receive an integrated assessment and
single plan. What services will need to work together,
strategically and at case level. What impact will this have on
the workforce?
 Pilot Project:
Working with 6 children whom have complex
needs across education, health, social care but don’t meet
thresholds for individual support. Starting October 2012.

Strong history of Parent Carer Engagement through the Aiming High
Programme

Parent Carers took a key role in developing the new CYPDS
service.......

Parent Carers said..... “the Jigsaw Service is great, but it works
with too few children. We want more children/young people to
benefit from services working together”

Southampton did.... “The new CYPDS virtually pulls together all
services from education, health and social care to work together to
make sure all children and young people with SEND are able to
achieve the life they want”

Parent Participation Plan:
 Project Manager and CYPDS Manager attend Abuzz Parent
Forum to consult as needed
 Parent Representatives are members of the SEND
Pathfinder Steering Group and Task and Finish Groups
 Wider consultation being undertaken through the Buzz
network as needed (over 500 parents)
 Support offered to all families engaged in the Pathfinder
 Quarterly Parent Carer newsletter to be launched in
October 2012

WHY DID WE WANT TO BE INVOLVED:
 Parent Carers know their children best – we saw this as an
opportunity to tell education, health and social care our
stories and directly influence how services are re-shaped
 We wanted to make sure services were working together –
we’re fed up of being in the middle!
 We wanted to highlight gaps and where services are
working well
