Background:

primary care team 367 clients on active register over 65 91 clients with dementia 2 agencies primarily providing service to this population • •

Problem:

Communication between primary Care team (PCT) and Alzheimer Society Ireland (A.S.I.) Identified through case experience and confirmed by questionnaire to primary care team members

Changes I Wished to Make

• • • • To improve communication between Alzheimer Society and Primary care team To implement a team approach to service planning for client To improve reporting on services client is receiving (home respite, day care, residential respite) To ensure proper communication between both teams with regards changes to services

The Plan

• • • • Questionnaire to members of primary care team.

Discuss change plan at clinical team meeting.

Meet with Coordinator of A.S.I to discuss communication.

Invite Coordinator to Clinical team meeting.

The Outcome

• • • • Following discussion and questionnaire the following issues were identified Communication problem. Result of questionnaire and meeting with A.S.I.

There is scope for improvement in the relationship between PCT and A.S.I

It is not feasible to have routine reporting on clients due to lack of resources but can take place on individual basis with service involved The care coordinator from the A.S.I. will attend clinical team meeting in Jan

Conclusion

• • This is an ongoing change. Improved understanding of each others service and expectations.
