Why Releasing Community Capacity

The Community Issues

• • • • • • • • • Gap in health inequalities are expected to widen between the most deprived and least deprived areas Ageing population, increasing isolation and frailty Hidden levels of depression due to lack of social interaction Main causes of death circulatory, cancers, heart disease and diseases of the respiratory system Obesity Smoking, including pregnant women Alcohol abuse and hospital admissions Increase in births and teenage conceptions. Higher rate of legal abortion. Higher infant mortality rate Lower rates of women accessing breast screening

The Community Issues

• • • • • • • • High numbers accessing benefits and lower than average hourly rate Reduced proportion of working adults with NVQL3 or equivalent and children achieving at least L2 writing, maths and science Low levels of participation in volunteering, adult sport and active recreation High rates of crime and the perception of crime Overcrowded housing in some deprived wards Increasing pressure from vulnerable groups Lack of suitable housing for some groups Problems reported accessing health services

Our Challenge: positive focus on well-being and health

• • • • • Promoting practical actions that will deliver improvement and change Moving from providing support in crisis interventions to tackling the underlying determinants of well-being Commissioning in new ways Supporting and providing community based solutions Managing the increased demand in a way that ensures fair and equitable service for all

The Programme

• • • • • Started in October 2012 Led by Board of community representatives and professionals Designed to build on what is already working well and to use the ‘assets’ in the community whether they are people or other resources A programme that tackles organisational knowledge, culture and behaviour by widening access to knowledge and skills in workers at the same time as increasing choice and service provision in the communities All residents and workers in communities: the ‘collective workforce’

The Role of the Change Champion • Supported by us • Complete 3 day course • Handbooks /workbooks/ handouts • Support local people and groups to identify gaps and generate new ideas • Access to funding via the HWB Board

Change Champions Stories

Change Champion Course Highlights

• A three day over three month intensive and practical course.

• Gives the participants the skills and confidence to set up and deliver interventions in their area.

• 11 Change Champion Cohorts have started up with almost 200 participants to date and a range of new initiatives. Cohorts have now graduated and are continuing to use their skills to bring about improvement and to support others.

• • • • • • • • • • Programmes they are working on: Time Banking Older People’s activities and lunch clubs Dementia education for practices, dementia friendly surgeries and a sleep pilot for people with dementia and their carers A reminiscence project Raising awareness of dementia services in the community Inter – cultural older people activities Developing a social enterprise to fund subsidised activities to develop sewing skills. Working with Mental Health services and the private sector Developing a young people’s clothing recycling company Information and awareness on diabetes Pulmonary rehab services for people in the community & a falls service in sheltered accommodation Change Champion graduates in calendar photo shoot.
