Many services, multi agencies and individuals working with the family Overlap and gaps in services Some families move back and forward through services Loads of practitioners going in and out of family Works well when they get the right relationship Bounce around lots of services, repeating their ‘story’ – no one seems to have the whole story Families enter into the system at different places which leads to different journeys Family context, rather than individual context needs to be understood

We know that some children and families face multiple problems, including:

Triage Service

The ‘Triage’ Service will become the unified ‘front end’ for accessing the full range of statutory processes, proactive and preventative services and for universal information, advice and guidance, ensuring that its work is characterised by high quality decisions at the appropriate speed.

Objectives

• • • Will become the point of contact for all requests for service across tiers 1-4 Will play a quality assurance role to ensure the quality of referrals into the services. Will screen and provide an initial decision to where the referral will be passed to.

Family Intervention Service

AIM

To create a new multi-agency holistic and integrated Family Intervention service for children across the 0-18 age range and their families. To ensure that children, young people and their families are able to access additional support services that are responsive to their needs early in a child’s life or in the onset or identification of difficulties at all levels of need.

What is a Prototype?

– Helps maintain service delivery while changes are being made – – Test new ways of working Small teams which will be built up over time

Prototype arrangements - Triage

What will Triage do?

• Receive and allocate all referrals and requests for service for High Need Families, Targeted Youth Support, Intensive Family Support, Early Years and Central Parenting • Provide information on Universal Services for Children and Families • Give advice and guidance on how to complete a CAF assessment correctly As of 12 March 2012 • All request for services for the above teams will need to be made to Triage team through a CAF assessment • Referrals will be allocated to: – Universal services/Provide information – – Hillingdon Families Service Statutory Services

Prototype arrangements – Hillingdon Families Service (HFS)

What will HFS do?

• Appropriate new cases will be allocated by Triage to Hillingdon Families Service • HFS will work with the whole family through key working with an early intervention approach Prototype • One member of staff from existing teams will be working together as a prototype team to test the new way of working • The new process will be reviewed constantly, will a full review at the end of June
