A comprehensive community effort to reduce infant mortality in the
Bootheel and St. Louis by mobilizing, facilitating, coordinating and
increasing engagement of multi-sector partners and community members

Collective Impact as a Framework
for Community Efforts
Common Agenda
All participants have a shared vision for change including a common
understanding of the problem and a joint approach
to solving it through agreed upon actions.
Shared Measurement
Collecting data and measuring consistently across all participants ensures
efforts remain aligned and participants hold each other accountable.
Mutually Reinforcing
Activities
Participant activities must be differentiated while still being coordinated
through a mutually reinforcing plan of action.
Continuous
Communication
Consistent and open communication is needed across the many players
to build trust, assure mutual objectives and create common motivation.
Backbone Support
Creating and managing collective impact requires a separate
organization(s) with staff and a specific set of skills to serve as the
backbone for the entire initiative and coordinate participating
organizations and agencies.

Implementation Grants
Purpose of Implementation Grants
• Surface new ideas to advance infant mortality reduction efforts in
focus communities
• Begin to engage partners in the IMRI
What We Look for in Grant Applications
• Projects will inform overall efforts
• Activities or projects that can create a measurable change within 12 months
• Fundable activities are not limited to the examples given in the RFA
—Be creative!
Commitment to Engage
• Learning is a hallmark of the IMRI
• Engagement to ensure transfer of learning
• Grantees will be required to engage in the IMRI:
 Attend a grantee orientation
 Attend a grantee convening
 Participate in meetings and conference calls
 Document and share data and action toward change

Eligibility Criteria
• Must meet MFH funding guidelines.
• One application per organization will be considered.
• Fiscal agents are allowed.
• Organizations must serve women, children and families or one or more of the
focus communities.
• Organizations from all sectors are encouraged to apply.
Ineligible Expenses
• Ongoing operating expenses for existing programs
• Major equipment (over $5,000)
• New staff positions
• Capital construction
Selection Process
• MFH staff and a team of community reviewers will review the applications
• Criteria outlined on pages 5−6 of the RFA
