Background
• July 2009 CHFG presented to the National
Patient Safety Forum
• The Forum concluded it needed to ‘better
understand’ human factors to benefit an
informed discussion ref relevance to safety
agenda
• Forum requested a Sub group comprising
CHFG reps and members of the Forum, to
scope the issues and report back

• Conclusion of Senior Leaders/opinion formers
‘ we don’t know, what we don’t know
• an informed debate is dependent on an
appreciation/understanding of the subject
•Without understanding Forum members will be
unable to see how HF relates to their spheres of
responsibility

Programme’ to the Forum

leaders
