Group Mediclaim Insurance Policy
• Mediclaim insurance scheme for pensioners and a few existing
employees and their spouses (and family pensioners) launched
from 01.01.2014.
• Coverage amount : Executives= Rs 7.5 lakhs p.a , Non-Executives
= 5 lakhs p.a
• The premium would be deducted from the pension in one go in
a given financial year according to the age profile.
• Admission to medical network facilities can either be planned or
on emergency both being cashless.
• Admission in any other medical facility outside tie up will be on
reimbursement basis.
• All preexisting diseases is covered from day one.

• Go to www.fhpl.net
• Click login and then click e-card

Enrollment for new pensioners
• Employees retiring every month to fill Form M
(GMIP).
• Respective Office/Administrative Officer/HR Officer
to send a copy of the duly filled up Form to the
Senior Manager (HR)-IR, DVC HQs .
• A time of 30-45 days would be taken after allotment
of PPO Number for coverage under the scheme.
Points to remember
a) Beneficiaries must carry photo ID proof along with medical
card for indoor treatment.
b) Beneficiaries who forget to carry medical card can download
electronic card from TPA website. Link of TPA is available in
DVC website. (TPA- Third Party Administrator)
c) Beneficiaries can choose to continue existing Mediclaim
Policy/Health Insurance and can use its benefits once DVC
GMIS is exhausted.
d) All relevant forms are available on the DVC website.
