ECDC: provide timely information to the Commission, the
Member States, Community agencies and international
organisations in the field of public health; rapid scientific
assessment of incoming alert messages;
Commission: Crisis management, measures

Health objectives and actions
ACTION STRANDS (6)
1) Enhance surveillance of threats
Surveillance of non communicable diseases
Co-ordination with ECDC
Laboratory network (rare/risky pathogens)
2) Respond effectively to threats
(new)
Improve preparedness for rapid intervention:
contingency planning, training, alert systems
Networking and best practice exchange
Health Threat Unit SANCO C3
New health programme
Health objectives and actions
3) Promote health tackling determinants
Life style/addictions: smoking, alcohol, nutrition, exercise, drugs
Environment and socio economic
4) Prevent diseases and injuries (new)
Complement determinants strand with action on major diseases: HIV,
cancer, cardiovascular and mental diseases
Reduce accidents and injuries
5) More and better Health information
Strengthen knowledge base
More dissemination
6) Achieve synergies between national health
systems (new)
Health technology assessment
Patient safety and mobility

Orientations 2006
Preparedness and response
Emphasis on preparedness. This heading could become
very large and possibly split in two.
Logistics, transportation issues
Influenza (e.g. preventive measures for veterinary workers
in case of Avian flu), in agreement with ECDC
Risk communication, EU network of public health media
officers, in agreement with ECDC
Linking hospitals to prepare for mass events
Traveller traceability (networking airports for contact tracing
and providing coordinated information to travellers)
Environmental sampling and analysis
Risk and threat analysis for health security incidents
Environmental (e.g. biosensors) and human biomonitoring
(e.g. for levels of bioagents in humans)
Psychological impact of health emergencies
Capacity building for joint law enforcement and health
authority operations
Health Threat Unit SANCO C3
Health Threat Unit
Orientations 2006
Providing risk management tools
Providing evidence-based information to
steer action (in co-ordination with ECDC)
Capacities needed for implementation
of IHR
IHR implementation in the EU
Health Threat Unit SANCO C3
Health Threat Unit
Orientations 2006
Strategies on vaccines and other drugs relevant
to communicable diseases control and Health
Security
Exchanging information on vaccination and immunisation
strategies
Exchanging information on other medicines (e.g.
antivirals)
Planning and policy implementation and strategies
related to addressing areas of decreasing or lower rates;
and in preparedness strategies (such as pre-event
vaccinations or stockpiling), especially for serious health
threats such as bioterrorism.
ECDC should be involved in all issues concerning the
scientific basis for planning and policies
Health Threat Unit SANCO C3
Health Threat Unit
Orientations 2006
Antimicrobial resistance and healthcare
associated infections – health services
aspects
Surveillance part of AMR will go to ECDC
but patient safety and healthcare related
actions may be required
impact on CD of mobility of patients
and healthcare staff
migrant health, cross-border
patient safety issues
Health Threat Unit SANCO C3
Health Threat Unit
Orientations 2006
Safety of blood, tissues and cells,
organs
Aims to promote the quality, safety and availability of
substances of human origin
Minimize risks and ensuing threats to patients’ health
associated with their collection, processing,
preservation, storage, distribution and use.
Activities should support implementation of existing EU
legislation

Generic preparedness planning
Focus of threats in the GPP
> outbreak
> local interest
Focus on 4 threats: biological , chemical, radiological,
environmental, deliberate or not
Major outbreak of an unknown illness (ex: SARS)
Previously unrecognised pathogen in the blood supply
Chemical, biological, or radiological contamination of a
water supply
Lost source or an accidental release of radiation
affecting >1MS
Serious imported infection affecting a number of countries
International concern over the safety of a vaccine
Next influenza pandemic
Suspected deliberate or accidental release of a serious
biological agent
Health Threat Unit SANCO C3
Health Threat Unit
Generic preparedness planning
Future orientations
Focus on role, functions and relations of public health partner
Set up a peer review process of national plans
To review the minimal requirements
To follow-up, test and exercise interoperability
Improve systematic information exchange and co-ordination
European measures in other sectors should be open to take into
account the public health aspect for countermeasures
COM will consider adopting relevant decisions on declaring
emergencies and on consultation of countermeasures
Propose algorithms, terminology and use of equivalent levels
Develop an EU-plan or process on triggering and building a
liaise and response structure
Continue the process
Health Threat Unit SANCO C3
Health Threat Unit
Generic preparedness planning
Basic concepts
Risk assessment
A scientifically based process consisting of the
following steps: I) hazard identification, ii) hazard
characterization, iii) exposure assessment, and iv)
risk characterization
Risk management
A process, distinct from risk assessment, of weighing
policy alternatives, in consultation with interested
parties, considering risk assessment and other
factors relevant for health protection of consumers
and for the promotion of fair trade practices, and if
needed selecting appropriate prevention and control
options.
Risk communication

Definitions
Public Health Threat
An event (incident) or condition involving one or
more agents, which has resulted in or has the
potential to produce rapidly harm, directly or
indirectly, to the health of an exposed population.
Public Health Crisis
A sequence of events following a public health threat,
where the limited time available for deciding and the
large degree of uncertainty leads to overburdening
the normal response capacity and/or undermining of
authority.

International aspects
Sanco’s traditional
international role
Involvement in international organisations
World Health Organization, OECD, CoE, G7+
Enlargement
10 countries joined in May 2004
Now new candidate/potential candidate
countries (Bulgaria, Romania, Turkey, Croatia,
Western Balkans)
Health Threat Unit SANCO C3
Sanco’s expanding
international role
Greater involvement in development of international
health treaties (FCTC, IHR)
“New” European Neighbourhood Programme Countries
Israel, Jordan, Moldova, Morocco, the Palestinian Authority,
Tunisia, Ukraine, Armenia, Azerbaijan, Egypt, Georgia,
Lebanon, Algeria, Belarus, Libya, Syria
Russia – Partnership and Cooperation Agreement, Four
“common spaces”.
A growing number of bilateral agreements with public
health element requested e.g. China, USA, India,
Thailand
Health Threat Unit SANCO C3
Health Threat Unit SANCO C3
Why? Globally
Health growing as a global foreign policy
concern
Global commerce and health
Recent wave of multilateral and bilateral free trade
agreements - exchange of goods and services that affect
health and health care
Disease and international security
Public health preparedness now more important than
during Cold War era
Prevention of bioterrorism
Awareness globalisation means epidemics such as SARS,
Avian Flu can spread more quickly
Role of diseases (e.g HIV/AIDS) in devastating
economies
