Information Sharing (IS) Index


A tool to help Practitioners to accurately identify:

the Child or Young Person before them

if they are receiving universal services

which other Practitioners are (or have been) involved with the child

that another Practitioner has information to share, has undertaken
an assessment or is taking action with regard to the Child
