Indicate whether the policy is to be mailed to the Agent or Owner.

Be sure to calculate premiums using the age as of the birthday .

Indicate the relationship of the Primary and Contingent Beneficiary to the Insured.

For bank drafts, include a voided check and complete the Request for Electronic Funds Transfer Plan on page 2 of the application. Select a draw date by checking one of the dates in “I request withdrawal of payments on or about the 1 st , 3 rd , 5 th , 10 th , 15 th , 20 th , or 25 th of each month.”

If the Owner requests that the first premium drafted from an account, indicate “0” under Amount Paid with Application (Section 4) and write “Draft First Premium” in Special Requests/ Remarks (Section 7).

Please mark either “yes” or “no” for the Automatic Premium Loan question (Section 4). The Automatic Premium Loan is not available in South Carolina.

Answer both questions regarding replacements in Section 6.

In Section 9, be sure to indicate City and State where the application was signed.

A telephone interview is required on Full Benefit Policies. When the interview is conducted from the Applicant’s home, it allows you to close the sale and greatly speed policy issue. Please refer to the Telephone Interview Brochure, Form No 4780CFG for details on how to arrange the interview.

30% Rule – Monitoring and Enforcement

• • • Application for Classic II GBL should not exceed 30% of the total number of Final Expense Plans submitted to Columbian If, at any time, your GBL percentage exceeds 30%, Columbian will inform you that you have 30 days in which to bring the percentage to an acceptable level or face suspension of the privilege to sell GBL. The company reserves the right to terminate anyone’s appointment at any time during the 30-day time period should the majority of applications remain GBL.
