1. Staff left individual in the
van for approximately 3-5
minutes while they went into
the bank to get individuals
funds for a shopping trip.
Individual has 24/7
supervision and is to be
within eye sight when in the
community. Provider reports
that staff could see the
individual from inside the
bank.
2. An individual went into
the break room in his
wheelchair and stopped at
the door blocking it so
others were unable to get
by. Staff yells across the
room “stop blocking the
door or I will come over
there and move you.”
