Adenocarcinoma of the esophagus & esophago-gastric junction

• Type I • Type II • Type III

?

Esophago-gastric resection

& gastric pull-up

Esophago-gastric resection

& gastric pull-up

Extended gastrectomy

& esophago-jejunostomy

Limited resection for early cancer :

short esophageal resection + proximal gastrectomy & Merendino jejunal interposition

Extended gastrectomy

(D2) & esophago-jejunostomy

Adenocarcinoma of the esophagus & esophago-gastric junction, Type II

Borderline location  • spesso considerati insieme: -

Tipo I + Tipo II Tipo II + Tipo III Tipo I + Tipo II + Tipo III

-

ca esofago + ca giunzione esofago-gastrica

• tipo II stadiato a volte come ca esofago e a volte come ca stomaco  Difficile valutare e confrontare i risultati ottenuti con le diverse tecniche chirurgiche

Strong individual preference and some degree of surgical mystique often govern selection of operation for resection

of GE junction adenocarcinoma ( Rusch 2004 )

The optimal surgical strategy remains controversial

Adenocarcinoma of the esophago-gastric junction,

Type II

Achieving R0 resection should be the goal of surgery (it is the most significant independent prognostic factor) problemi ancora controversi relativi all’intervento chirurgico

•

via di accesso

•

volume di resezione esofagea

•

volume di resezione gastrica

•

estensione della linfoadenectomia

The type of operation selected may affect: the ability to achieve a complete (R0) resection, the quality of lymph node staging, the long-term chance of local control, and patient’s quality of life

Adenocarcinoma del cardias Problemi controversi relativi all’intervento chirurgico

•

( sec. Pinotti )

– resezione esofago addominale per via laparotomica esclusiva

Adenocarcinoma del cardias Problemi controversi relativi all’intervento chirurgico

•

volume di resezione gastrica:

–

resezione gastrica polare superiore

Type II

adenocarcinoma of the E-G junction

Esophago-gastric resection

& gastric pull-up

Extended gastrectomy

& esophago-jejunostomy
