This is the Landing Screen of the Employee Status Form (ESCF).
The employee initiating his/her ESCF form would be required to fill in his / her Employee No to continue
Post validation of the Emp-No, the employee is redirected to the main ESCF Form which is divided into the
following parts:
 Top Row displaying the time remaining to complete the form before the current session times out
 Header consisting of
 Emp-No
 Name
 Signature of Originator : Signum of Employee & Current date
 Effective Date of Change (To be filled by the Employee)
 Left Section : Displays all the current attributes of an employee that can be changed through an ESCF
Request
 Right Section : Displays all the available attributes of an employee that can be changed through an ESCF
Request
The employee can view his/her current attributes in the left section and select the attributes to be modified
from the right Section (Eg : Designation , Grade etc)
Once the employee has finished populating all the rows in the right section (Mandatory Fields), he/she should
fill the employee comments (Reason for ESCF) and press the Submit Button at the Bottom
On successful submission of the form, the employee would get the confirmation screen
A successful ESCF submission would result in the following mail being sent to the employee automatically
with the URL and the Key to track the ESCF request online
Simultaneously another auto email is triggered to the old manager of the employee intimating him/her about
the ESCF initiation with the URL and key needed to approve / decline the ESCF request
Once the manager follows the URL , he/she is requested to key in the Request No as received in the e-mail
Post validation of the Key-No, The manager is redirected to the main ESCF form as filled by the employee
All the fields in this page are un editable and only the option to Approve / decline the ESCF case is available
to the manager with Remarks. The can change the highlighted colums.
Once manager fills the recommendation he/she should click the submit button at the bottom of the page
As soon as the manager clicks the Submit button, and email is triggered to the old Circle Head for action.
The mail consists the URL and Key Id
Once the CH clicks the URL as received in the email, he/she would be prompted to enter the Request No as
received in the email
Post validation of the Key-No, The CH is redirected to the main ESCF form as filled by the employee
The Recommendation and date of action of the Manager is also visible on the form
All the fields in this page are un editable and only the option to Approve / decline the ESCF case is available
to the CH with Remarks
Once CH fills the recommendation he/she should click the submit button at the bottom of the page
As soon as the CH clicks the Submit button, and email is triggered to the New Manager for action.
The mail consists the URL and Key Id (Request-No)
Post validation of the Key-No, The new manager is redirected to the main ESCF form as filled by the
employee
The recommendation of Old Manager and Old CH are visible on the form
All the fields in this page are un editable and only the option to Approve / decline the ESCF case is available
to the manager with Remarks
Once manager fills the recommendation he/she should click the submit button at the bottom of the page
As soon as the new manager clicks the Submit button, and email is triggered to the New Circle Head for
action.

Once the New CH clicks the URL as received in the email, he/she would be prompted to enter the Request No as
received in the email

Once the HR Verification clicks the URL as received in the email, he/she would be prompted to enter the
Request No as received in the email
Once the HR Verification clicks the URL as received in the email, he/she would be prompted to enter the Request
No as received in the email
Post validation of the Key-No, The HR Verification is redirected to the main ESCF form as filled by the employee
The Recommendation and date of action of the Old Manager, Old CH, New Manager are also visible on the form
All the fields in this page are un editable and only the option to Approve / decline the ESCF case is available to
the HR Verification with Remarks
Once the HR Verification fills the recommendation he/she should click the submit button at the bottom of the page
Once the HR Approver clicks the URL as received in the email, he/she would be prompted to enter the Request
No as received in the email
Once the HR Approver clicks the URL as received in the email, he/she would be prompted to enter the Request
No as received in the email
Post validation of the Key-No, The HR Approver is redirected to the main ESCF form as filled by the employee
The Recommendation and date of action of the Old Manager, Old CH, New Manager are also visible on the form
All the fields in this page are un editable and only the option to Approve / decline the ESCF case is available to
the HR Approver with Remarks
Once the HR Approver fills the recommendation he/she should click the submit button at the bottom of the page
Once the ESCF is completed duly by all the stakeholders, the changes are automatically updated in the Essjay
HRMS so as to reflect the new employee attributes.
Also the employee would receive the Transfer letter as attached below
