Accident Location: Rig 36 Lekhwair
WHAT HAPPENED: The IP was working on the iron roughneck which had been previously removed from the drill floor as
bolts holding the pistons to the roughneck body had sheared. With the use of a webbing strap, the IP was supervising
reintroducing the top assembly of the roughneck onto its base assembly and engaged the back pin successfully into its
slot. The IP then attempted to rotate the upper assembly to allow it to drop down onto the two front pins. In doing so the IP
placed his fingers in a 4cm gap below the upper assembly pistons and as it successfully dropped on to the pins the gap
reduced to less than a 1cm. The very top of his right ring finger was severely crushed.

down further tightening the wire rope and causing his hands to become trapped between the pulley and the wire rope.
IP was immediately taken to the clinic for first aid treatment for a cut and loss of nail to his left index finger. He was later
referred by the Lekhwair clinic to Ibri hospital where he was diagnosed with a fractured left index finger.
