• From April 2013 Family support Teams and
Social Work Teams have relocated into area
hubs.
• Co locating Family support workers and Social
Workers together has created a greater
cohesion between teams alongside extending
links to local services and identifying support for
families that qualify under the “Troubled Family “
agenda.
