Themes:
• Families, parents and young people are at the heart of the changes. Person centred
approach;
• Streamlined, integrated (coordinated) assessment leading to an outcome focused
EHCP from 0-25;
• Services must be commissioned jointly between the LA (education and social
care/adults and children's) and health;
• Local Offer must capture everything that is normally available and what is additional;
criteria to access it
• 0-25 provision with statutory protection and redress to SEND Tribunal;
• Personal budgets can be requested to extend choice and control;
• All schools will be included in primary legislation on SEN and MUST comply.

Children and young people still have a right to be educated in a mainstream school
Local authorities must still consider parents’/young people’s preference when
naming a place of education on the EHC plan
Child’s rights under the Equality Act 2010
Provision must still be as specific and quantified on EHC plans
EHC plans must be reviewed at least once a year.
Parents /young people right to appeal to the Tribunal* against all statutory decision
Local Structure
Education Health Care Plan (EHC Plan)
Agreed processes and report templates, which include:
Diagnosis /current concerns, medical information, cognition and learning, communication
and interaction, social, mental and emotional wellbeing , sensory /physical and finally
existing intervention /management
• Desired Outcomes (what this will mean for…………..)
• What ………needs to support him/her
• How often this will need to happen and how will it be reviewed
• Who will provide the support?
• How will we know if the outcome is being met

Stage 1
Weeks 0-6
A request for an EHC
assessment is
submitted by
educational setting or
parents
Parent Partnership
will contact parents
to offer a meeting to
explain services
available to them
including the process
and the Local Offer.
A Multi Agency Panel
will be held to decide
if an assessment is
needed based on the
criteria set by
Education, Health and
Social Care.
If an EHC assessment
is not necessary the
case will be referred
to the Early Help
Service. Parents have
the right to appeal at
this stage.
Stage 2
Weeks 6-12
Parents will meet with
an SEN Team member
who will explain what
will happen during the
assessment period.
Parents will provide
written advice which
forms part of the
assessment process
Professionals involved
with parents or their
child will be identified;
this may be done in the
meeting and contacted
to submit reports.
Stage 3
Weeks 12-16
All information collected in stage
2 will be submitted to the Multi
Agency Panel. It will be decided if
an EHC Plan is necessary:

An EHC Plan production meeting
will be arranged to start
composing the plan. The plan will
be drafted by the SEN Team and
your Caseworker working with
the parent/young person and the
professionals involved.
A draft EHC plan will be sent to
parents/young person and
professionals to check and
approve. Parents/young person
have the chance to express a
school preference at this stage.
If an EHC Plan is not necessary:

the case will be referred to the
Early Help Service. Parents have

Stage 4
Weeks 16-20
The SEN Team and
your Caseworker will
work with you to
create a final EHC
plan.
Once Finalised you
will be sent a final
copy of the EHC plan
and all professionals
will receive a copy.
If you are not in
agreement with the
EHC Plan you have
the right to appeal.
The EHC Plan will be
reviewed annually.

The law calls for all Councils to keep in one place all information that a parent or
young person with SEND might need to access/know within the age range of 0-25.
Information on all services in the borough and available in neighbouring boroughs.
It includes information on health, education and social care. Including activities
outside of school, where to access advice, support and guidance.
It clearly shows how to access services and how these are funded.
It gives information on all of the changes happening locally such as how to apply for
an EHC and Personal Budgets.
Parents and Young People can provide feedback and help to shape the way we
commission services in the council.

Designed, implemented and managed by the councils Parent Partnership Team.
This strategic decision facilitates the use of local knowledge about what parents ask
for, want and need.
All teams and managers have a responsibility to share ownership in the offer and to
contribute to ongoing improvements to services.
We consider this a ‘web-based’ resource which can be accessed by families and by
staff when they are working with families.
Increased staff and extended hrs to assist families who require some support to
access the offer
A simple on line feedback system and termly events at the CDC. Feedback will be
given to managers to inform planning and commissioning.
