Self Assessment



EWP Self Assessment form located
on the “Z” drive under DOCS section
Each employee must be afforded an
opportunity to provide the supervisor
with a self-assessment of his or her
job performance for the rating period
Self-assessment is voluntary by the
employee. Employee should notify
supervisor in writing if chooses not to
complete self-assessment.

Evaluating the Employee

Supervisor must review and consider
the self-assessment when
completing each employee’s
performance evaluation

Supervisors are encouraged to
discuss the self-assessment with the
employee before completing the
employee's performance evaluation

Evaluating the Employee


Supervisor uses the “Supervisor’s File” for
preparing the employee’s evaluation.
The “Supervisor’s File” contains
documentation of:






Work performance - quality & quantity
Attendance, attitude, work with others
Counseling notes
Originals of notifications with plans for
improvement
Copies of written notices
(The file should be available to the employee)

Transfers

If an employee leaves his/her
position during the performance
cycle, the previous supervisor should
complete an interim evaluation

After 6 months into the cycle, if an
employee transfers to another
agency a full performance evaluation
is completed

Supervisor/Reviewer

The supervisor completes the
evaluation and discusses the
evaluation with the reviewer.
Reviewer can change the evaluation
if necessary

The supervisor and reviewer agree
on an overall rating and both sign
and date the evaluation form

Meeting With the Employee



The supervisor schedules a
meeting with the employee to
review the performance
evaluation
Employee signs the evaluation
form
The original of the entire EWP
form is forwarded to HR by Nov
1st

Ratings - Contributor


Contributor – The employee is
performing the full range of core
responsibilities and core
measures
The majority of employees will
receive this rating

Ratings – Extraordinary
Contributor

Extraordinary Contributor - Results or
work that characterized by exemplary
accomplishments throughout the rating
period; performance that is considerably
and consistently well above performance
measures

Usually work outside of normal expected
performance. Extraordinary Contributor
form must be attached to the Performance
Evaluation

Contributor

Supervisors are encouraged by
policy to recognize incidents of
employee performance that truly are
extraordinary

The receipt of one or more
Acknowledgement of
Extraordinary Contribution form does
not automatically entitle an employee
to an overall annual rating of
“Extraordinary Contributor”

Ratings – Below Contributor

Below Contributor – Results or work
that fails to meet performance
measures

To receive this rating the employee
must have one or more “Notice of
Improvement of Substandard
Performance” form(s) with attached
plan for improvement (improvement
must occur within 30 to 180 days)
attached to the form

Can lead to termination or
demotion and can be used in
conjunction with written notices
If demotion is the result there is
a mandatory 5% decrease in
salary
No salary increase with this
rating

Ratings – Below Contributor

The supervisor must provide an
Improvement Plan within 10
working days

The employee is re-evaluated 2
weeks before the 3 month
improvement period ends

Probationary Employees

Probationary employees will be
rated

Supervisors have the option of
using the Probationary Progress
Review or the full performance
evaluation (those completing 12
months must receive full
evaluation)

Who does the Evaluation?



The immediate supervisor must
complete the evaluation section of
the evaluation form
The instructions are located in the
DOCS section of the “Z” drive- EWP,
Instructions for Completion
New supervisor - The new
supervisor is responsible for
completing the official performance
evaluation with input from the
previous supervisor

Appeal Process



Employee may appeal
evaluation rating
Appeal must be made in writing
to the reviewer within 10 days of
the evaluation conference
Reviewer should provide a
response within 5 working days

Response to Appeal

The Reviewer’s Choices:





Reviewer agrees with the evaluation
and upholds the original rating
Asks supervisor to revise the evaluation
Asks supervisor to complete new
evaluation
Reviewer revises the evaluation
Reviewer completes new evaluation

Next Performance Cycle



Employee and supervisor develop
EWP for FY 2012
EWP must be signed by supervisor,
employee & reviewer. Signed
original to HR no later than
November 18th.
Original retained in HR will be
returned to supervisors for
completion by the supervisor and
reviewer in Fall 2012

Physical Requirements of Job


Completion of the last page of
EWP form (Appendix E) is
required for every position
Submit with completed 2012
EWP to HR Office
