IMPORTANT REMINDERS
Statistics Requirement
• You will be required to fulfill a basic statistics requirement as a part of the MSW program. You may wish
to enroll in an introductory statistics course either through the School of Social Work's Continuing
Education Program, or at a local community college.
Confidential Disclosure Form
• Each applicant is required to complete the Confidential Disclosure Document. THE CONFIDENTIAL
DISCLOSURE DOCUMENT IS NOT USED TO DETERMINE ADMISSIONS ELIGIBILITY. Fieldwork
sites often require students to submit to background checks as a prerequisite for acceptance. These
background checks can be extensive and students need to be aware that arrests/convictions may affect
acceptance for some fieldwork sites and/or the future ability to become licensed. We ask for this
information so students are appropriately matched with fieldwork sites.
The Confidential Disclosure Document information is considered highly confidential and private and is
not released or shared with faculty, field instructors, or any fieldwork site personnel.
Please state all issues honestly and thoroughly to avoid serious consequences including a delay in
beginning a field placement, rejection by a fieldwork site, and/or the inability to progress with your MSW
fieldwork requirements. You should be aware that willful omission or misrepresentation on any part of
your application including the Confidential Disclosure Document may be grounds for revocation of
admission.
