and evaluate the local Safe Routes to School
(SRTS) plan.

 Identify and work with one 3rd-5th elementary school.

Walk project (meet monthly).
Recruit parents/students to complete on-line survey (by end of Sept).
Coordinate Teacher Tally and enter results on line (last wk Sept).
Attend, facilitate and recruit volunteers for GPS Walkability
Assessment (late Sept/Oct).
Provide compensation to schools for parent/child survey.
Promote attendance at community presentation (May/June).
Complete an on-line monitoring survey (every month).
Provide feedback on program, training, support materials.

Compensation
Each Local Public Health will receive $4500 for contract
duties. Payments will be issued after completion of
designated activity.
