Designed to enable people to experience improved emotional wellbeing
and promote early recovery from emotional problems e.g. low mood,
stress and anxiety
Includes Link Workers and IAPT joined by partner organizations from the
3rd sector (voluntary and charitable)
Seamless integration of wellbeing, mental health, acute hospital,
secondary community and primary care ensuring people experience a
smooth transition
Anyone age 13 plus experiencing emotional health problems and
registered with a GP in Suffolk
Focus on individuals to address their own wellbeing needs, build their
own resilience and support systems
