FORM 1006 – SURETY BOND FORM 1006 – SURETY BOND This presentation will assist you in the completion of the Form 1006. The Date of Last Revision: 2003 Rule Requirement: OAC 165:10-1-12 Explanation of Form: Prior to drilling and/or operating a well, the operator shall furnish the Conservation Division a surety bond ($25,000.00) or other present alternate surety, Form 1006A or 1006C. Operator must file the original copy only with a copy of the power of attorney from the bonding company. The name and address of the Oklahoma resident service agent shall be endorsed on the bond form. [Reference 165:10-1-10 and 165:10-1-12] Question concerning this Form: Contact the Surety Department (405) 521-2246 FORM 1006 – SURETY BOND OTC/OCC OPERATOR NUMBER – leave this space blank if you are a new operator and filing for surety the very first time FORM 1006 – SURETY BOND Bond Number – this number is given by the company that is underwriting the bond FORM 1006 – SURETY BOND Will this bond be for continuous coverage or issued for a specified time period (OCC Rules requires all sureties to be issued for not less than a one year period at a minimum) FORM 1006 – SURETY BOND NAME OF OPERATOR – the name of the operator must the name that is filed on the Form 1006B and at the Oklahoma Secretary of State FORM 1006 – SURETY BOND ALL OPERATORS ARE REQUIRED TO PROVIDE A PHYSICAL ADDRESS EVEN IF THE OPERATOR RECEIVES THEIR MAIL AT A POST OFFICE BOX FORM 1006 – SURETY BOND PHONE NUMBER, FAX NUMBER, A CONTACT PERSON AND E-MAIL ADDRESS ARE REQUESTED FORM 1006 – SURETY BOND SURETY COMPANY INFORMATION FORM 1006 – SURETY BOND AMOUNT OF SURETY – blanket coverage is $25,000, if any amount less the operator needs to get a Form 1006D completed by a licensed well plugger and submit it along with the Form 1006 and Form 1006B FORM 1006 – SURETY BOND SIGNATURE BLOCK – Both the Surety Company Representative and a Representative of the Oil and Gas Operator must sign this document. Only originals will be accepted. No faxed or emailed copies will be accepted.
