Enter your email address and password and select
Login to enter the system

To view report
click on download

Users of MIS Reports can select from 24 months of reports
Click on Data Selector

To change the time period of the report
Select year of report
to view
Select month of report to view
Select “Get Report”
After selecting “Get Report” you will be taken back to the screen to download the report

The MIS Report information page opens in a new window
LOGOUT
Select to exit the Information Services
Portal
You will be logged out of the Information Services Portal and directed back to the
LOGIN page.
If you do not submit any requests for 30 minutes the system will time out and you
will automatically be logged out of the system.
If the Information Services Reports Portal is disconnected due to a system failure
you will automatically be logged out of the system.
