Notice Total Monthly Payment Due.
Notice Statement at the Top of Bill and the Coupon at the Bottom of the Bill.
Notice Perforations to Allow Bill to be Separated.
Place “Accepted For Value at 45 Degree Angle on the Statement Part of the Bill in “Red Ink”.
Write the Money Order on the coupon.
“Write-In” the Amount You Want to be Utilized to Pay the Bill.
Sign in “Blue Ink” with “Authorized Representative” Written Directly Underneath Your Signature.
“Return For Value”, And “For Settlement & Closure” are options, depending on what you want to do.
Stamps and thumbprints are optional FRONT OF STATEMENT/BILL

Using thumbprints is an option, not a necessity
