– * 3.00 pm to 6.00 pm for collecting certificates/documents/reports

The LDC summarily looks through the papers If there is any patent and apparent
shortcoming the paper is immediately returned for rectification

THE SUGAM SINGLE WINDOW PROCESS……..
If found okay the application form is handed over to the computer operator who enters the
service type, name and generates the slip which heaving proposed date of delivery
Computer Operator gives the Applicant a slip and sticker copy of the same receipt is stuck
to the application and handed over to the UDC/SW Incharge
The SW Incharge minutely goes through the application. If any shortcoming is found then
the appropriate remarks are made and the application is kept in the ‘Incomplete’ tray
otherwise it is temporarily kept in the ‘Complete’ tray
At 3.00 PM, the operator enters the information of all the applications in the ‘Complete’ tray
and takes out printouts of the Certificates. This process can also be carried throughout the
day whenever the opportunity arises
The completed Certificates alongwith the attached documents are sent to the desk of the
designated officers in a ready for signature state.
Alongwith the certificates, an automatically generated cover letter is also sent with the
details of the applications. This is to serve as an office record of the concerned officer. A
copy of the same is maintained at the Sugam SW

The designated officer prepare the certificates, documents or compliance report and deliver it before the
stipulated time of service delivery
On the stipulated time of service delivery, as mentioned in the client’s receipt, the certificate, document or
compliance is delivered to the citizen on production of deposit receipt
The SWS has an inbuilt continuous monitoring system. These reports bring about the progress in terms
of number of applications received, desposed, pending with in time limit and pending beyond time limits
DOMICILE CERTIFICATE
Requirements:
• Application form in prescribed format.
• Affidavit in prescribed format on pie paper.
• Educational documents detailing last ten years of studies.(if educated)
• Documents indicating continuous residence for 10 years – voter list,
electricity/water bill etc.
• Ration card photocopy attested by gazetted officer.
• Application form to be attested by
Sarpanch and Patwari (for Rural)
Nagar Nigam/Palika - Parshad/Member (for Urban)
• Application form attested by two gazetted officers (rural and urban)
• Three Passport Size Photographs
OTHER BACKWARD / SPECIAL BACKWARD CLASS CERTIFICATE
Requirements:
• Affidavit on pie paper in prescribed format.
• Application form to be certified by
Sarpanch and Patwari for Rural area
Nagar Nigam/Palika – Parshad/member and gazetted officer for Urban area
• Income certificate and copy of jamabandi (for those above land ceiling limit)
• For government servants, DDO’s certificate in prescribed format.
• Caste by Birth Certificate photocopy attested by gazetted officer
• Domicile certificate photocopy attested by gazetted officer
• Patwari’s report
• Attested photocopy of Ration Card
• Three Passport Size Photographs
SC/ST CERTIFICATE
Requirements:
• Application form in prescribed format attested by one gazetted officer.
• Affidavit in prescribed format on pie paper
• Ration card photocopy attested by gazetted officer.
• Application form to be attested by

Nagar Nigam/Palika - Parshad/Member for Urban
• Caste by Birth Certificate
• Patwari’s report
• Four Passport Size Photographs
Old Age/Widow/Physically Handicapped Pension
Requirements:
• Application form in prescribed format.
• Certificate of Annual Income
• Age Proof
• Death Certificate of husband for Widow
• BPL card photocopy attested by gazetted officer
• Physically Handicapped certificate photocopy attested by gazetted officer
• Affidavit of guardian if handicap is minor regarding income of applicant
• Ration card photocopy attested by gazetted officer.
• Report of Sarpanch and Patwari (for Rural)
• Report of Nagar Nigam/Palika - Parshad/Member and Inspector- Nagar Nigam/
Palika (for Urban)
• Five Passport Size Photographs
