Non-CPS Student Eligibility
Letter Request Form
• Fill out form and return to Mrs. Heybach
• Forms sent in by Oct 1 will allow students
to test on Oct. 26 or Nov 23 without filling
out an application first
• Test scores will be sent 3 weeks later
• Students will be able to apply to schools
knowing their total point score
• All forms come through Mrs. Heybach
