Key Components
CSA PerformCare is the Contracted Systems Administrator for CSOC - Single portal for access to care CMO A care management organization that serves children, youth, young adults and their families/caregivers with high and moderate level of care coordination needs FSO Family-led support for CMO involved families, community education, warm lines, advocacy MRSS Crisis planning for youth with behavioral/emotional needs, available 24/7/365
PerformCare is Contracted Systems Administrator (CSA) for CSOC
CSA functions
• Serve as single point of access for youth to Children’s System of Care •Anyone may contact PerformCare on behalf of a youth;
however, parent/legal guardian must give consent before services can be initiated
• Authorize services, based on most recent clinical information submitted • Provide 24-hour/day, 7 days/week availability • Coordinate access to services for eligible youth • Help youth obtain any necessary specialized behavioral health services and/or services for intellectually/developmental disabled •
CSA does not provide direct services

Child Family Team (CFT)
Team of family members, professionals, and community residents organized by the case management entity to design and oversee implementation of Individual Service Plan
Individual Service Plan (ISP)
Comprehensive, strength-based plan developed by Child/Family Team that integrates clinical and social services and supports with family and community resources to address needs of child and family across life domains in order to support the child, youth, or young adult to remain in or return to the community, in school and out of trouble

Children’s System of Care Intellectual/Developmental Disabilities Supporting children with intellectual disabilities/developmental disabilities (ID/DD)
As of January 1, 2013 the Children’s System of Care: • Manages all services for children with ID/DD • Provides services including family support services (e.g. respite) and residential services •

Children’s System of Care Substance Abuse Services for Children and Adolescents Supporting children with substance abuse challenges
As of July 1, 2013 the Children’s System of Care: • Authorizes youth who meet specific criteria to receive substance use treatment services from any of 15 service providers contracted with the CSOC • Provides services including outpatient, intensive outpatient, partial care, short & long term residential treatment, and sub-acute detoxification 13

Ages 13 up to 18 th birthday New Jersey residents Meet criteria for substance abuse treatment based on standardized assessment (American Society of Addiction Medicine or ASAM) Other means of funding (e.g. private insurance) exhausted Youth actively involved with: • Division of Child Protection & Permanency (DCP&P) • • Juvenile Court (identified Court/Probation agents) Care Management Organization (CMO) • Mobile Response & Stabilization Services (MRSS ) Families and youth may also contact PerformCare directly to confirm eligibility for services 14
Children’s System of Care Pathways to Collaboration County Inter-Agency Coordinating Council (CIACC)
• CIACC • County-based organization comprised of local public and private child-serving agencies, families/caregivers, and youth • • • Exist in all 21 counties Identify local needs and service gaps Advise DCF on development and maintenance of a responsive, accessible, and integrated system of care • CIACC Educational Partnership • Cross-systems training and network-building initiative to increase collaboration among schools, DCF, and other public and private child-serving agencies • Help ensure children and families are connected to child protective, behavioral health, developmental disability, addiction, and other services quickly and efficiently 15
