• One of the earliest known written laws
• Babylonian laws and punishments addressed safety
 If a builder builds a house for someone, and does not construct it properly, and
the house which he built falls in and kills its owner, then the builder shall be put
to death.
• One of the earliest building codes…although pretty harsh it did incentivize focus on
safety
• Only qualified builders, prepared to take the risk of death, built houses
• There may have been a shortage of subcontractors in ancient Babylon
• We don’t kill subcontractors; however, 4,000 years later LATA KY still takes safety
seriously
Be A Good Safety Citizen
• Use your influence
• Demonstrate personal commitment to do the right thing

Adding Vendors to the
Approved Supplier List (ASL)
•
Suppliers of critical items are evaluated and approved prior to subcontract or
purchase order award. Quality Assurance (QA) is responsible for determining if a
supplier is capable of meeting applicable requirements described in Title 10 CFR
Part 830.122, Quality Assurance Criteria, and/or PAD-PLA-QM-001, Quality
Assurance Program and Implementation Plan for the Paducah Environmental
Remediation Project.
•
Information on the vendor/supplier (QA) Program or Plan is requested on the
Prospective New Supplier Questionnaire
– Indicate whether the company ‘s Quality Manual and/or implementing procedures
address the elements in DOE 414. 1C Quality Program Elements and provide
documentation showing use of the elements (i.e., QA procedures, QA manuals)
– Identify the codes and/or Standards to which the company QA Program complies
(i.e., certificate numbers, dates of certification)
– Provide information regarding the organizational structure of the company (i.e.,
organizational chart, count of personnel in QA)
– Identify the different equipment available and activities performed by the company
(i.e., welding, national codes/standards routinely used for manufacturing/processing
products or services)

Evaluation for Addition to ASL
Supplier evaluation and selection may include one or more of the
following:
• Supplier history of providing identical or similar product that
performs satisfactory in actual use;
• Supplier quality record information and practices that can be
evaluated objectively through written documents and recognized
certification(s); and
• Supplier's technical and quality capability as determined by direct
evaluation of facilities, personnel, and implementation of supplier's
QA program.
An on-site assessment of the supplier may be performed before a final
determination is made for addition to the ASL.
IF the supplier is not able to meet the requirements of a stand-alone
QA program, bidders may be directed to work under the LATA
Kentucky QA program for work performed on-site.

Maintaining Approved Supplier Status
Annually QA sends out “Annual Supplier Evaluation
Questionnaire” to vendors/suppliers requesting the following
information:

Changes made to the company QA Program in the past 12 months
If certificates or accreditations from nationally recognized organizations,
such as ASME, ISO, NAVLAP, ASNT, AWS, etc., have been maintained
• Has the company QA Program been audited in the past year by other state
or federal agencies or listed as an approved supplier for any NQA-1
regulated facility and/or DOE prime operating contractor in the last 12
months
• Have any significant programmatic deficiencies been identified in the past
year either by internal or external audit sources
LATA Kentucky evaluates end user satisfaction through Supplier Service
Evaluation and reviews supplier nonconformances for the year.

Critical Items Defined
Critical items are goods and services, including commercial grade items that
require rigorous procurement and inspection processes to prevent significant
personal injury to the workforce and public and/or an environmental
noncompliance. Critical items include, but are not limited to, the following:
• Waste containers (e.g., shipping containers, intermodals, drums, waste
boxes) for off-site shipment
• Sorbent or absorbent materials used in waste packaging
• Waste treatment equipment and waste treatment services
• Waste disposal services for regulated wastes
• M&TE and M&TE calibration services and calibration standards (i.e.,
buffers, weights, etc.)
• Instrument calibration services
• Hoisting and rigging equipment and hardware
