毕业证书
• 报读者需要向考试机构申请官方成绩
单，并由该机构发送至香港中文大学
研究院
• 若成绩不理想可再次报考，并把合乎
最要求的成绩单再次发送至研究院

并开具现阶段已有的官方成绩单

• 两封推荐信
• 及早申请港澳通行证 (不需提交)
• 准备好可用于网上支付的信用卡

如已通过相关英语考试，并取得满意成绩的申请者可选填相应的英语成绩；
若还未参加考试或者成绩未出，可暂时不填。此处多种英语成绩类型，只
需填写其中一种即可，如同时有多种考试的成绩，全都填上也无妨
网上申请-职业资格证书及工作经历

此处填写申请者的相关职业资格证书，不是填写语言证书或大学奖项，
而是如司法考试、执业医师、注册会计师等。此栏空着也没关系。
此处填写工作或实习经历，从最近的经历往前写，若没有也可不填。

这是关于信息来源的问卷调查，即你是通过什么途径知道这个硕士学位
项目的，根据实际情况选择即可。

网上申请-网上支付报名费
这里选择付款方式，报名费为300港币。
如果持有支持visa/master的信用卡可直接网上付款，这是最方便的；
若没有则需要选择第二种“汇票”（Bankdraft/Cheque）的方式，并
将汇票随同最后的申请材料寄送至学校。

 填完所有表格，核对无误后，单击表
单”Submit”
 提交成功后，系统会生成个人专属的
申请编号Application Number，一定要
牢记此编号！往后填表格及再次登入
申请系统修改均需要用到此编号
 注意：整个填表时间需控制在1小时
内，故正式填表前需浏览熟悉完整的
表格

通过此系统修改，需要发邮件至研究
生院才能修改，所以填写时避免错误

递交申请后，如有任何修改可凭申请编号及所填写的居民身份证号登录更改。




绩单一同放入需要寄送的材料中。

注意：成绩单需由学校盖章，并装
在信封内，信封上下两个封口均要

邮寄材料

需要邮寄的材料如下：
1. 学位证及毕业证复印件（若为在读学生，则邮寄在读证明原件）
2. 大学官方成绩单原件（需密封）及成绩单cover sheet

3. 英语成绩单复印件 （寄送材料时如还未取得合格的英语成绩单可暂不寄送英
语成绩单，研究院或会给予conditional offer – “条件性录取”，开学前能提交
合格的成绩单，即有机会正式录取）
4. 身份证复印件 （身份证正反面复印在同一张纸上）
5. 个人简历
6. 推荐信两封（需密封）
7. 网上报名阶段如用汇票或支票付款，需将银行汇款单原件也一起附上；若使
用信用卡网上付款，则不需要。

邮寄材料放入寄送的包裹前，最好先放入一大文件袋或大信封内，并用单独的一
张纸写上申请姓名、申请编号， 并逐条列出信封中有哪些材料。
