 2. For projects completed in Last 3 years.
 Re-certify by Exam in lieu of points

IF CERTIFICATION EXPIRES
 Points will NOT be accepted to regain
certified status except in the case of

 3. Pay a fee of $50.00 for extension
 4. Must be submitted by Dec. 31st
